Community Health Assessment (Link Below)

Your Voices Matters! Please complete the NC8 Public Health Coalition Community Health Survey!

The Norfolk County-8 Public Health Coalition, a group of towns in Norfolk County that share resources and collaborate to improve public health, is conducting a community health assessment. This assessment will help gain a greater understanding of the health concerns, priorities, and challenges facing residents in our region about issues that matter most—whether it’s access to care, chronic disease, mental health, environmental health or other topics.

We are asking community members to give us your feedback and suggestions about health in your community by completing this 10-minute survey. This survey is open to people who live or work in the towns of: Canton, Dedham, Norwood, Walpole, Westwood.

Filling out the survey is voluntary, and your responses are anonymous; there are no right or wrong answers. It’s your opinion that matters! At the end of the survey, you will find information on how you can enter a raffle for the chance to win a $50 gift card.

Take the survey using this link: https://bit.ly/nc8survey

The survey is available in 3 languages: English, Portuguese and Spanish.
